LIVING WILL FORMS AVAILABLE AT BOSCOLA’S OFFICE

BETHLEHEM (Mar 22) – State Senator Lisa Boscola announced today that living will forms are available at her offices by visiting either her Bethlehem office at 559 Main Street – Suite 270, Whitehall office at 2123 North First Avenue, or the Stroudsburg office located at 600 Main Street – Suite 112.  Living wills can also be obtained by calling (610) 868-8667 or toll-free 1-877-535-1818.

As in most states, the Pennsylvania “living will” law extends to terminally ill and/or permanently unconscious adults the right to death with dignity.  In deciding whether to make a living will declaration, one should consult with one or more of the following: family, doctor, clergy and/or attorney.

Included in the brochure available from Senator Boscola’s office is a living will form that is based on the suggested sample contained in Pennsylvania’s law.  That does not mean that a living will needs to be in this form or that it can’t include other specific directions.
